    Cuidado Personal Care

    Our Personal Care Program provides services to persons who need help with day-to-day activities to allow them to be more independent in their own home. A personal care assistant is an individual trained to help persons with basic daily routines. Personal Care is also referred to as ” Primary Home Care”, ” Personal Care Assistance” and / or ” Primary Care Option”.

    Personal Care Services must be medically necessary and ordered by a physician. A person must need help to complete activities of daily living, have health-related tasks that they need help with or be in need of observation and/ or redirection of behavior.

    State Based Programs

    Primary Home Care (PHC)

    Primary Home Care (PHC) is a Texas based program. PHC provides non-technical in-home attendant services to people who have an approved medical need for assistance with personal care tasks. PHC is available to eligible people whose health problems cause them to be functionally limited in performing activities of daily living according to a practitioner’s statement of medical need. Attendants provide PHC services.

    Who can get services?

    • You must be 21 or older.
    • You must have an unmet need for help with home management and personal care tasks.
    • You must have a practitioner’s statement of medical need.
    • You must be eligible for Medicaid.
    • You must meet specified functional assessment score and have a functional limitation with at least one personal care task based on your medical condition.

    Primary Care Option

    Primary Care Option  is a New Mexico based program.  PCO is a program designed to assist certain Medicaid eligible consumers to obtain non-medical assistance at home with activities of daily living (ADLs) determined by HSD-approved level of care (LOC) criteria. The PCO program offers a range of services to consumers who are unable to perform some or all activities of daily living because of a disability or functional limitation. PCO services permit an eligible consumer to live in his or her home rather than in an institution.

    Who is eligible?

    • Must have full Medicaid benefits
    • Must be 21 years of age and over
    • Must meet the State-approved medical eligibility requirements

    Personal Care Assistance

    Personal Care Assistance is a Virginia based program.  A Personal Care Assistant (PCA) helps you with some or all of your Activities of Daily Living (ADLs) and other tasks identified in your support plan. PCA provides non-technical in-home attendant services to people who have an approved medical need for assistance with personal care tasks. This program is available to eligible people whose health problems cause them to be functionally limited in performing activities of daily living according to a practitioner’s statement of medical need. Attendants provide PHC services.

    Who is eligible?

    • Must first be eligible for Medicaid or you must be using a Medicaid waiver.
    • Must be 21 years of age and over
    • If you are 60 years of age or older, contact your local Area Agency on Aging. If you have a disability, contact your local Community Service Board or Center for Independent Living. These agencies may be able to provide assistance with personal care or assistance services.
